 Enrollment Forms 

• After passing probation – CUPE 382 New Enrollment
• Add a spouse, Overage Dependent, or baby – CUPE 382 Addition of Eligible Dependents; if > 4 months after a life event please,
  complete a Late Dependents Application form

Notice of Conversion: for those resigning or retiring who wish to convert to an individual plan 

• Retirees have access to the district benefits plan until the end of the one month past their retirement date 
• For those who resign or take severance, benefits are effective until the end of the month your last day of work occurred.
